At 78, Trump is already the oldest man ever elected president. However, he would still be younger than President Joe Biden, who concluded his term at the age of 82. The doctor’s emphasis on Trump’s activity level and fitness could be a key narrative in his campaign to portray vitality and energy in contrast to Biden.

Cognitive and Mental Health: Trump ‘Fully Fit’ in Cognitive Tests

Trump again passed the Montreal Cognitive Assessment (MoCA), a widely used screening tool for cognitive function. It includes memory recall, attention, and language comprehension tasks. This is the same test Trump took in 2018, famously recalling a list of words—“Person. Woman. Man. Camera. TV”—which he once used to demonstrate his cognitive sharpness.

The latest health summary confirmed that Trump scored well on screenings for depression and anxiety, suggesting sound mental health in addition to physical wellness. According to Barbabella, Trump’s daily schedule includes multiple high-pressure engagements, media events, and “frequent victories in golf,” all indicative of a sharp and active mind.

Trump’s cholesterol levels have significantly improved, falling from 223 in 2018 to just 140 in 2025. This improvement is largely attributed to his ongoing treatment with rosuvastatin and ezetimibe, both cholesterol-lowering medications.

His blood pressure was recorded at 128/74, which is slightly elevated, but it’s not considered dangerous.

The doctor noted that proactive measures are being taken to monitor and control it.

Trump also maintains a resting heart rate of 62 beats per minute, signaling good cardiovascular fitness, especially for someone his age.

Medical History: Cataract Surgery, Colonoscopy, and Skin Examination

The report included a few previously undisclosed medical details. Trump has had cataract surgery on both eyes, although the dates were not provided. Such procedures are common for seniors and typically restore clarity in vision through lens replacement.

In July 2024, Trump underwent a colonoscopy that revealed a benign polyp and a condition called diverticulosis—a weakening of the intestinal wall that is common with aging but generally asymptomatic. No abnormalities were reported in this area, and the polyp was successfully removed.

The exam also revealed minor sun damage to Trump’s skin and a few benign lesions, but no concerning growths. Scarring on his right ear—stemming from the July 2024 assassination attempt at a Butler, Pennsylvania, rally—was also noted.

Barbabella is a Purple Heart and Legion of Merit recipient, with multiple tours in Iraq and Afghanistan, and now serves as the third consecutive osteopathic physician to hold the role under both Democratic and Republican administrations.

Barbabella emphasized that Trump is “fully fit to execute the duties of Commander-in-Chief and Head of State.” He specifically cited Trump’s “robust” health and his ability to manage a demanding public schedule, signaling strong capacity for a return to the Oval Office.

Trump’s Fitness Compared to Biden’s Age

While concerns about age and mental acuity have surrounded both Trump and Biden, this recent physical report clearly positions Trump as healthy and ready to take on presidential duties. Biden, now 82, has also faced public scrutiny regarding his physical stamina and cognitive sharpness, but Trump’s team is using his clean bill of health to contrast the two leaders.

If re-elected in November, Trump would begin his term at age 79, overtaking his own record as the oldest elected president. However, based on this recent medical exam, he is reportedly in better shape now than during his first term.
